First published in 1970, The Primal Scream was a book that escaped the clinic, entered popular culture, and turned “primal therapy” into shorthand for the belief that adult neurosis is rooted in repressed childhood pain.

Janov’s argument is that early emotional injury does not disappear; it is buried, defended against and reorganised into symptoms, habits and distorted forms of living. Therapy, in his account, therefore cannot remain purely interpretive. The patient must return to the original pain, experience it bodily and emotionally, and break through the defensive structures built around it. The famous “scream” is less a gimmick in Janov’s own account than a possible expression of this release. Might not seem revelatory now, but then, it certainly was

Historically, the book is as interesting for what it reveals about the era as for the theory itself: post-Freudian psychology colliding with counterculture, somatic release, anti-authoritarian psychiatry and a growing suspicion of polite, endlessly verbal therapy. Much of Janov’s method is now heavily contested, but the book remains an extraordinary artefact of therapeutic radicalism and the 1970s hunger for a cure that felt total.
